The ability to customize default fields in ZM contact and appointment forms

Would love to be able to customize the default fields that are hardcoded to ZM and allow conditional formatting to each field if necessary. For example, if the marketing source is a referral, it would be nice to have the option to conditionally format the field to pop up another field for the referral contact's name.
